Prevent Blindness Tennessee is excited to be working with Vision Service Plan to provide free vision care to Tennessee's children. Please help us identify children that need this service. If you know of a child who:
- is 18 years old or younger and
- does not have vision insurance or TennCare and
- whose family cannot afford to pay for an eye exam and glasses,
please print the information and application forms through the link below:

Read the instructions first, and be sure the child qualifies for the program. Then the parents or teacher need to fill the form out completely and fax it or send it to us at the following address:
