Senior Sight Outreach Program Referral Application
Senior Sight Outreach (SSO) is a three-pronged program that provides education, screenings, free exams and low cost glasses to Tennessee’s uninsured, indigent, and working poor by forming partnerships with local community-based agencies that already serve these populations in their areas.
Adult Vision Screenings
Prevent Blindness screens many seniors each year in an effort to eliminate preventable blindness in Tennessee. A screening does not take the place of a professional eye examination, but it can help identify individuals at risk for eye disease. Finding eye disease in its early, treatable stages saves sight. Prevent Blindness’ screenings consists of a number of steps: risk assessment questions about medical history, age, race and most recent eye appointments, screen for refractive problems (nearsightedness and farsightedness), and a visual field test to help detect significant changes in your peripheral vision. Lastly, the screener discusses screening results with each person and makes a recommendation regarding the need for professional eye care.
Diabetic Retinopathy Intervention Program (DRIP) Screenings
PBT conducts vision clinics throughout Davidson and surrounding counties to provide digital retinal screenings for retinopathy and makes referrals for patients to see a retinal specialist, as needed. Patients, over the age of 40, who are identified as diabetics or pre-diabetics, by health centers and primary care centers receive digital retinal screenings to detect early diagnosis of diabetic retinopathy. If the retinal scan indicates that the client has a serious eye condition related to diabetic retinopathy, PBT will facilitate a referral to an ophthalmologist for treatment as well as prevention education. Prevent Blindness Tennessee will follow-up with the referring organizations and clients to track the outcomes of treatment received.
Eye Exam and Eyeglasses Program
After determining those in need of services, clients are invited to our in office clinic where an eye doctor provides on-site eye exams and an optician measures and fit glasses. Prescription glasses are provided for $50.00 to $75.00 a pair depending on the prescription. Prevent Blindness dispenses the glasses approximately 2 weeks later. This program occurs monthly. If the exam indicates that the client may have a serious eye conditions affecting his or her vision, the eye doctor will make an immediate referral to a local physician or agency.
