Healthy Eyes Eyeglass Program and Sight for Student applications are now online!

PBA & Healthy Eyes
Prevent Blindness America, the nation’s oldest eye health and safety organization, has partnered with Healthy Eyes, a Luxottica Group Foundation dedicated to improving vision through outreach, research and edu-cation, to provide free eyeglasses frames and prescription lenses at locations across the country. The Prevent Blindness America “Healthy Eyes Eyeglass Program” provides cards to individuals that can be utilized through the OneSight program at any participating locations:

  • LensCrafters
  • Sears Optical
  • Target Optical
  • Pearle Vision

It will be up to the voucher recipient to contact his/her desired store to ensure the location is participating.
This program is available to children and adults. Applications are accessible online at our website or call or office 800-850-2020 to request one today.

Eligibility Requirements

  • Applicant must have no other eyeglass insurance benefits or is not eligible at time of application.
  • Applicant has not used program during the last 12 months.
  • Applicant must have current eyeglass prescription (less than 2 years old). This program does NOT cover an eye examination.
  • Family income is no more than 200% of poverty level (see chart below).

Sight for Students

Sight for Studentsis a program founded by Vision Service Plan (VSP). The program provides vision exams and glasses to uninsured children across the country.

Vision Service Plan (VSP), a national healthcare leader in the area of eye care services, is partnering with Prevent Blindness America to provide eye exams and eyeglasses for uninsured or underinsured children throughout the nation.

Vision Service Plan (VSP) is the nation’s oldest and largest managed vision care plan offering services through a network of private practice providers (both optometrists and ophthalmologists) that offer quality eye care services.  There are more than 20,000 VSP doctors located in all 50 states and Puerto Rico.

This program is available only to students who meet The Eligibility Requirements.

Eligibility Requirements

  • Family income is no more than 200% of poverty level (see chart below)
  • Child is not enrolled in Medicaid and does not have any vision insurance
  • Child is 18 years old or younger, and is still attending school
  • Child or parent is a U.S. citizen or documented immigrant with a social security number
  • Child has not used our program during the last 12 months.
