Prevent Blindness Nebraska asks you to focus:

Become a FOCUS Group Sponsor and watch as others are given the opportunity to see clearly!

20/20 Donation Level - $2,000

  • One four-hour employee/customer screening and two four-hour community screenings
  • Sponsorship of one Operation Eyeglasses, a comprehensive vision clinic for the homeless.
  • Your company/organization name, logo and marketing materials displayed at the screenings.

20/30 Donation Level - $1,500

  • One four-hour employee/customer screening and two four-hour community screenings
  • Your company/organization name, logo and marketing materials displayed at the screenings.

20/40 Donation Level - $1,000

  • One four-hour employee/customer screening and one four-hour community screening
  • Your company/organization name, logo and marketing materials displayed at the screenings.

20/60 Donation Level - $500

  • One four-hour employee/customer screening OR One four-hour community screening
  • Your company/organization name, logo and marketing materials displayed at the screening.

SCREENING NEEDS AND INFORMATION

  • For children’s screening we need 10 feet of space for a vision chart
  • For adults, we need a six-foot table with access to electricity and four to six chairs.
  • Adult screening consists of the following:
    • Visual Acuity: A machine-generated chart, which indicates at what level (20/20-20/200+) an individual is capable of seeing. We request current glasses or contacts be worn.
    • FDT Visual Fields: A machine that displays waves of light and movement throughout all areas of visual fields, including peripheral vision, which is most often affected by glaucoma.
    • Completion of a registration form for all participants.
  • The total time needed per person, including completion of the form and a brief exit interview, is approximately five minutes. The screening may take longer depending on age, ability to understand directions and the ability to see adequately.
